2. Educational Background

Applicants must provide one of the following:

  • Equivalent foreign credential (evaluated if applicable)

3. Government-Issued Identification

  • Valid Driver’s License, State ID, or Passport
  • Social Security Card (if required for clinical placement)

4. Background Check

  • Criminal background screening may be required for programs involving clinical or externship placement.
  • Certain criminal offenses may impact eligibility for certification or employment in healthcare.

5. Immunizations & Health Requirements

  • TB test (PPD or Quantiferon)
